Dear Edwyn: This is not an answer to your question, rather it is to suggest you do a little leg work on the Internet. I did a ten minute "sweep" but did not find any site which offers an adaptor patch suiting your need.
Hopefully some other member has a helpful solution to offer. Good luck. Keep well.
